3a6455d56b mm: convert do_brk_flags() to use vma_flags_t
In order to be able to do this, we need to change VM_DATA_DEFAULT_FLAGS
and friends and update the architecture-specific definitions also.

We then have to update some KSM logic to handle VMA flags, and introduce
VMA_STACK_FLAGS to define the vma_flags_t equivalent of VM_STACK_FLAGS.

We also introduce two helper functions for use during the time we are
converting legacy flags to vma_flags_t values - vma_flags_to_legacy() and
legacy_to_vma_flags().

This enables us to iteratively make changes to break these changes up into
separate parts.

We use these explicitly here to keep VM_STACK_FLAGS around for certain
users which need to maintain the legacy vm_flags_t values for the time
being.

We are no longer able to rely on the simple VM_xxx being set to zero if
the feature is not enabled, so in the case of VM_DROPPABLE we introduce
VMA_DROPPABLE as the vma_flags_t equivalent, which is set to
EMPTY_VMA_FLAGS if the droppable flag is not available.

While we're here, we make the description of do_brk_flags() into a kdoc
comment, as it almost was already.

We use vma_flags_to_legacy() to not need to update the vm_get_page_prot()
logic as this time.

Note that in create_init_stack_vma() we have to replace the BUILD_BUG_ON()
with a VM_WARN_ON_ONCE() as the tested values are no longer build time
available.

We also update mprotect_fixup() to use VMA flags where possible, though we
have to live with a little duplication between vm_flags_t and vma_flags_t
values for the time being until further conversions are made.

While we're here, update VM_SPECIAL to be defined in terms of
VMA_SPECIAL_FLAGS now we have vma_flags_to_legacy().

Finally, we update the VMA tests to reflect these changes.

   - Completely new memory management. s390 was a special snowflake that
     enlisted help from the architecture's page table management to
     build hypervisor page tables, in particular enabling sharing the
     last level of page tables. This however was a lot of code (~3K
     lines) in order to support KVM, and also blocked several features.
     The biggest advantages is that the page size of userspace is
     completely independent of the page size used by the guest:
     userspace can mix normal pages, THPs and hugetlbfs as it sees fit,
     and in fact transparent hugepages were not possible before. It's
     also now possible to have nested guests and guests with huge pages
     running on the same host

   - Maintainership change for s390 vfio-pci

   - Small quality of life improvement for protected guests

  x86:

   - Add support for giving the guest full ownership of PMU hardware
     (contexted switched around the fastpath run loop) and allowing
     direct access to data MSRs and PMCs (restricted by the vPMU model).

     KVM still intercepts access to control registers, e.g. to enforce
     event filtering and to prevent the guest from profiling sensitive
     host state. This is more accurate, since it has no risk of
     contention and thus dropped events, and also has significantly less
     overhead.

     For more information, see the commit message for merge commit
     bf2c3138ae ("Merge tag 'kvm-x86-pmu-6.20' ...")

   - Disallow changing the virtual CPU model if L2 is active, for all
     the same reasons KVM disallows change the model after the first
     KVM_RUN

   - Fix a bug where KVM would incorrectly reject host accesses to PV
     MSRs when running with KVM_CAP_ENFORCE_PV_FEATURE_CPUID enabled,
     even if those were advertised as supported to userspace,

   - Fix a bug with protected guest state (SEV-ES/SNP and TDX) VMs,
     where KVM would attempt to read CR3 configuring an async #PF entry

   - Fail the build if EXPORT_SYMBOL_GPL or EXPORT_SYMBOL is used in KVM
     (for x86 only) to enforce usage of EXPORT_SYMBOL_FOR_KVM_INTERNAL.
     Only a few exports that are intended for external usage, and those
     are allowed explicitly

   - When checking nested events after a vCPU is unblocked, ignore
     -EBUSY instead of WARNing. Userspace can sometimes put the vCPU
     into what should be an impossible state, and spurious exit to
     userspace on -EBUSY does not really do anything to solve the issue

   - Also throw in the towel and drop the WARN on INIT/SIPI being
     blocked when vCPU is in Wait-For-SIPI, which also resulted in
     playing whack-a-mole with syzkaller stuffing architecturally
     impossible states into KVM

   - Add support for new Intel instructions that don't require anything
     beyond enumerating feature flags to userspace

   - Grab SRCU when reading PDPTRs in KVM_GET_SREGS2

   - Add WARNs to guard against modifying KVM's CPU caps outside of the
     intended setup flow, as nested VMX in particular is sensitive to
     unexpected changes in KVM's golden configuration

   - Add a quirk to allow userspace to opt-in to actually suppress EOI
     broadcasts when the suppression feature is enabled by the guest
     (currently limited to split IRQCHIP, i.e. userspace I/O APIC).
     Sadly, simply fixing KVM to honor Suppress EOI Broadcasts isn't an
     option as some userspaces have come to rely on KVM's buggy behavior
     (KVM advertises Supress EOI Broadcast irrespective of whether or
     not userspace I/O APIC supports Directed EOIs)

   - Clean up KVM's handling of marking mapped vCPU pages dirty

   - Drop a pile of *ancient* sanity checks hidden behind in KVM's
     unused ASSERT() macro, most of which could be trivially triggered
     by the guest and/or user, and all of which were useless

   - Fold "struct dest_map" into its sole user, "struct rtc_status", to
     make it more obvious what the weird parameter is used for, and to
     allow fropping these RTC shenanigans if CONFIG_KVM_IOAPIC=n

   - Rework KVM's handling of VMCS updates while L2 is active to
     temporarily switch to vmcs01 instead of deferring the update until
     the next nested VM-Exit.

     The deferred updates approach directly contributed to several bugs,
     was proving to be a maintenance burden due to the difficulty in
     auditing the correctness of deferred updates, and was polluting
     "struct nested_vmx" with a growing pile of booleans

   - Fix an SGX bug where KVM would incorrectly try to handle EPCM page
     faults, and instead always reflect them into the guest. Since KVM
     doesn't shadow EPCM entries, EPCM violations cannot be due to KVM
     interference and can't be resolved by KVM

   - Fix a bug where KVM would register its posted interrupt wakeup
     handler even if loading kvm-intel.ko ultimately failed

   - Disallow access to vmcb12 fields that aren't fully supported,
     mostly to avoid weirdness and complexity for FRED and other
     features, where KVM wants enable VMCS shadowing for fields that
     conditionally exist

   - Print out the "bad" offsets and values if kvm-intel.ko refuses to
     load (or refuses to online a CPU) due to a VMCS config mismatch

  x86 (AMD):

   - Drop a user-triggerable WARN on nested_svm_load_cr3() failure

   - Add support for virtualizing ERAPS. Note, correct virtualization of
     ERAPS relies on an upcoming, publicly announced change in the APM
     to reduce the set of conditions where hardware (i.e. KVM) *must*
     flush the RAP

   - Ignore nSVM intercepts for instructions that are not supported
     according to L1's virtual CPU model

   - Add support for expedited writes to the fast MMIO bus, a la VMX's
     fastpath for EPT Misconfig

   - Remove kvm_gmem_populate()'s preparation tracking and half-baked
     hugepage handling. SEV/SNP was the only user of the tracking and it
     can do it via the RMP

   - Retroactively document and enforce (for SNP) that
     KVM_SEV_SNP_LAUNCH_UPDATE and KVM_TDX_INIT_MEM_REGION require the
     source page to be 4KiB aligned, to avoid non-trivial complexity for
     something that no known VMM seems to be doing and to avoid an API
     special case for in-place conversion, which simply can't support
     unaligned sources

   - When populating guest_memfd memory, GUP the source page in common
     code and pass the refcounted page to the vendor callback, instead
     of letting vendor code do the heavy lifting. Doing so avoids a
     looming deadlock bug with in-place due an AB-BA conflict betwee
     mmap_lock and guest_memfd's filemap invalidate lock

  Generic:

   - Fix a bug where KVM would ignore the vCPU's selected address space
     when creating a vCPU-specific mapping of guest memory. Actually
     this bug could not be hit even on x86, the only architecture with
     multiple address spaces, but it's a bug nevertheless"

8e38607aa4 treewide: provide a generic clear_user_page() variant
Patch series "mm: folio_zero_user: clear page ranges", v11.

This series adds clearing of contiguous page ranges for hugepages.

The series improves on the current discontiguous clearing approach in two
ways:

  - clear pages in a contiguous fashion.
  - use batched clearing via clear_pages() wherever exposed.

The first is useful because it allows us to make much better use of
hardware prefetchers.

The second, enables advertising the real extent to the processor.  Where
specific instructions support it (ex.  string instructions on x86; "mops"
on arm64 etc), a processor can optimize based on this because, instead of
seeing a sequence of 8-byte stores, or a sequence of 4KB pages, it sees a
larger unit being operated on.

For instance, AMD Zen uarchs (for extents larger than LLC-size) switch to
a mode where they start eliding cacheline allocation.  This is helpful not
just because it results in higher bandwidth, but also because now the
cache is not evicting useful cachelines and replacing them with zeroes.

Demand faulting a 64GB region shows performance improvement:

 [*] This improvement is because switching to sequential clearing
  allows the hardware prefetchers to do a much better job.

 [#] For pg-sz=1GB a large part of the improvement is because of the
  cacheline elision mentioned above. preempt=full|lazy improves upon
  that because, not needing explicit invocations of cond_resched() to
  ensure reasonable preemption latency, it can clear the full extent
  as a single unit. In comparison the maximum extent used for
  preempt=none|voluntary is PROCESS_PAGES_NON_PREEMPT_BATCH (32MB).

  When provided the full extent the processor forgoes allocating
  cachelines on this path almost entirely.

  (The hope is that eventually, in the fullness of time, the lazy
   preemption model will be able to do the same job that none or
   voluntary models are used for, allowing us to do away with
   cond_resched().)

Raghavendra also tested previous version of the series on AMD Genoa and
sees similar improvement [1] with preempt=lazy.

This patch (of 8):

Let's drop all variants that effectively map to clear_page() and provide
it in a generic variant instead.

We'll use the macro clear_user_page to indicate whether an architecture
provides it's own variant.

Also, clear_user_page() is only called from the generic variant of
clear_user_highpage(), so define it only if the architecture does not
provide a clear_user_highpage().  And, for simplicity define it in
linux/highmem.h.

Note that for parisc, clear_page() and clear_user_page() map to
clear_page_asm(), so we can just get rid of the custom clear_user_page()
implementation.  There is a clear_user_page_asm() function on parisc, that
seems to be unused.  Not sure what's up with that.

6a35d02fec s390/vmem: Support 2G page splitting for KASAN shadow freeing
Export split_pud_page() so it can be used from the vmem code and teach
modify_pud_table() to split PUD-sized mappings when only a subrange
needs to be removed.

If the range to be removed covers a full PUD-sized mapping, keep the
existing behavior: clear the PUD entry and free the backing large page
(for non-direct mappings). Otherwise, split the PUD-mapped page into
PMD mappings and let the walker handle the smaller ranges.

This is needed for KASAN early shadow removal support: memory hotplug
freeing the KASAN early shadow is the only expected caller that will
try to free 2G PUD-mapped regions of non-direct mappings.

A rather large series which is supposed to fix the crash below[1], which was
seen when running the memop kernel kvm selftest.

Problem is that cmpxchg_user_key() is executing code with a non-default
key. If a system is IPL'ed with "LOAD NORMAL", and in addition the previous
system used storage keys where the fetch-protection bit is set for some pages,
and the cmpxchg_user_key() is located within such page a protection exception
will happen when executing such code.

Idea of this series is to register all code locations running with a
non-default key at compile time. All functions, which run with a non-default
key, then must explicitly call an init function which initializes the storage
key of all pages containing such code locations with default key, which
prevents such protection exceptions.

Furthermore all functions containing code which may be executed with a
non-default access key must be marked with __kprobes to prevent out-of-line
execution of any instruction of such functions, which would result in the same
problem.

By default the kernel will not issue any storage key changing instructions
like before, which will preserve the keyless-subset mode optimizations in
hosts.

Other possible implementations which I discarded:

- Moving the code to an own section. This would require an s390 specific
  change to modpost.c, which complains about section mismatches (EX_TABLE
  entries in non-default text section). No other architecture has something
  similar, so let's keep this architecture specific hack local.

- Just apply the default storage key to the whole kprobes text
  section. However this would add special s390 semantics to the kprobes text
  section, which no other architecture has. History has shown that such hacks
  fire back sooner or later.

Furthermore, and to keep this whole stuff quite simple, this only works for
code locations in core kernel code, not within modules. After this series
there is no module code left with such code, and as of now I don't see any new
kernel code which runs with a non-default access key.

Note: the original crash can be reproduced by replacing

page_set_storage_key(real, PAGE_DEFAULT_KEY, 1);

with

page_set_storage_key(real, 8, 1);

in arch/s390/kernel/skey.c:__skey_regions_initialize()

ac5bf06650 s390/page: Add memory clobber to page_set_storage_key()
Add memory clobbers to the page_set_storage_key() inline assemblies. This
allows for data dependencies from other code, which is important to prevent
the compiler from reordering instructions if required.

Note that this doesn't fix a bug in existing code; this is just a
prerequisite for upcoming code changes.

42398caf16 s390: Replace __ASSEMBLY__ with __ASSEMBLER__ in non-uapi headers
While the GCC and Clang compilers already define __ASSEMBLER__
automatically when compiling assembler code, __ASSEMBLY__ is a
macro that only gets defined by the Makefiles in the kernel.
This is bad since macros starting with two underscores are names
that are reserved by the C language. It can also be very confusing
for the developers when switching between userspace and kernelspace
coding, or when dealing with uapi headers that rather should use
__ASSEMBLER__  instead. So let's now standardize on the __ASSEMBLER__
macro that is provided by the compilers.

This is a completely mechanical patch (done with a simple "sed -i"
statement), with some manual fixups done later while rebasing the
patch.

03544866df s390/mm: Add configurable STRICT_MM_TYPECHECKS
Add support for configurable STRICT_MM_TYPECHECKS. The s390 ABI defines
that return values with complex types like structures and unions are
returned in a return value buffer allocated by the caller. This is also
true for small structures and unions which would fit into a register.  On
the other hand when such types are passed as arguments to functions they
are passed in registers, if they are small enough.
This leads to inefficient code when such a return value of a function call
is then passed as argument to a subsequent function call.

This is especially true for all mm types, like pte_t and others, which are
only for type checking reasons defined as a structure. This however can be
bypassed with the STRICT_MM_TYPECHECKS feature, which is used by a few
other architectures, which seem to have the same problem.

Add CONFIG_STRICT_MM_TYPECHECKS which can be used to change the type of
pte_t and other structures. If the config option is not enabled the types
are defined to unsigned long, allowing for better code generation, however
there is no type checking anymore. If it is enabled the types are
structures like before so that type checking is performed, but less
efficient code is generated.

The option is always enabled in debug_defconfig, and for convenience an
mmtypes.config topic target is added, which allows to easily enable it, in
case memory management code is changed.

CONFIG_STRICT_MM_TYPECHECKS and STRICT_MM_TYPECHECKS are kept separate,
since STRICT_MM_TYPECHECKS is common across architectures and common
code. Therefore use the same define also for s390 code.

Add CONFIG_STRICT_MM_TYPECHECKS to make it build time configurable.

c5c3238d9b asm-generic: provide generic page_to_phys and phys_to_page implementations
page_to_phys is duplicated by all architectures, and from some strange
reason placed in <asm/io.h> where it doesn't fit at all.

phys_to_page is only provided by a few architectures despite having a lot
of open coded users.

Provide generic versions in <asm-generic/memory_model.h> to make these
helpers more easily usable.

Note with this patch powerpc loses the CONFIG_DEBUG_VIRTUAL pfn_valid
check.  It will be added back in a generic version later.

1642285e51 s390/boot: Fix KASLR base offset off by __START_KERNEL bytes
Symbol offsets to the KASLR base do not match symbol address in
the vmlinux image. That is the result of setting the KASLR base
to the beginning of .text section as result of an optimization.

Revert that optimization and allocate virtual memory for the
whole kernel image including __START_KERNEL bytes as per the
linker script. That allows keeping the semantics of the KASLR
base offset in sync with other architectures.

Rename __START_KERNEL to TEXT_OFFSET, since it represents the
offset of the .text section within the kernel image, rather than
a virtual address.

Still skip mapping TEXT_OFFSET bytes to save memory on pgtables
and provoke exceptions in case an attempt to access this area is
made, as no kernel symbol may reside there.

In case CONFIG_KASAN is enabled the location counter might exceed
the value of TEXT_OFFSET, while the decompressor linker script
forcefully resets it to TEXT_OFFSET, which leads to a sections
overlap link failure. Use MAX() expression to avoid that.

99b3f8f76f s390/uv: Implement HAVE_ARCH_MAKE_FOLIO_ACCESSIBLE
Let's also implement HAVE_ARCH_MAKE_FOLIO_ACCESSIBLE, so we can convert
arch_make_page_accessible() to be a simple wrapper around
arch_make_folio_accessible(). Unfortunately, we cannot do that in the
header.

There are only two arch_make_page_accessible() calls remaining in gup.c.
We can now drop HAVE_ARCH_MAKE_PAGE_ACCESSIBLE completely form core-MM.
We'll handle that separately, once the s390x part landed.

036c0e104b s390/uv: Convert PG_arch_1 users to only work on small folios
Now that make_folio_secure() may only set PG_arch_1 for small folios,
let's convert relevant remaining UV code to only work on (small) folios
and simply reject large folios early. This way, we'll never end up
touching PG_arch_1 on tail pages of a large folio in UV code.

The folio_get()/folio_put() for functions that are documented to already
hold a folio reference look weird; likely they are required to make
concurrent gmap_make_secure() back off because the caller might only hold
an implicit reference due to the page mapping. So leave that alone for now.

9ecaa2e94e s390: Relocate vmlinux ELF data to virtual address space
Currently kernel image relocation tables and other ELF
data are set to base zero. Since kernel virtual and
physical address spaces are uncoupled the kernel is
mapped at the top of the virtual address space, hence
making the information contained in vmlinux ELF tables
inconsistent.

That does not pose any issue with regard to the kernel
booting and operation, but makes it difficult to use a
generated vmlinux with some debugging tools (e.g. gdb).

Relocate vmlinux image base address from zero to a base
address in the virtual address space. It is the address
that kernel is mapped to in cases KASLR is disabled.

The vmlinux ELF header before and after this change looks

56b1069c40 s390/boot: Rework deployment of the kernel image
Rework deployment of kernel image for both compressed and
uncompressed variants as defined by CONFIG_KERNEL_UNCOMPRESSED
kernel configuration variable.

In case CONFIG_KERNEL_UNCOMPRESSED is disabled avoid uncompressing
the kernel to a temporary buffer and copying it to the target
address. Instead, uncompress it directly to the target destination.

In case CONFIG_KERNEL_UNCOMPRESSED is enabled avoid moving the
kernel to default 0x100000 location when KASLR is disabled or
failed. Instead, use the uncompressed kernel image directly.

In case KASLR is disabled or failed .amode31 section location in
memory is not randomized and precedes the kernel image. In case
CONFIG_KERNEL_UNCOMPRESSED is disabled that location overlaps the
area used by the decompression algorithm. That is fine, since that
area is not used after the decompression finished and the size of
.amode31 section is not expected to exceed BOOT_HEAP_SIZE ever.

There is no decompression in case CONFIG_KERNEL_UNCOMPRESSED is
enabled. Therefore, rename decompress_kernel() to deploy_kernel(),
which better describes both uncompressed and compressed cases.

Introduce AMODE31_SIZE macro to avoid immediate value of 0x3000
(the size of .amode31 section) in the decompressor linker script.
Modify the vmlinux linker script to force the size of .amode31
section to AMODE31_SIZE (the value of (_eamode31 - _samode31)
could otherwise differ as result of compiler options used).

Introduce __START_KERNEL macro that defines the kernel ELF image
entry point and set it to the currrent value of 0x100000.

54f2ecc318 s390: Map kernel at fixed location when KASLR is disabled
Since kernel virtual and physical address spaces are
uncoupled the kernel is mapped at the top of the virtual
address space in case KASLR is disabled.

c98d2ecae0 s390/mm: Uncouple physical vs virtual address spaces
The uncoupling physical vs virtual address spaces brings
the following benefits to s390:

- virtual memory layout flexibility;
- closes the address gap between kernel and modules, it
  caused s390-only problems in the past (e.g. 'perf' bugs);
- allows getting rid of trampolines used for module calls
  into kernel;
- allows simplifying BPF trampoline;
- minor performance improvement in branch prediction;
- kernel randomization entropy is magnitude bigger, as it is
  derived from the amount of available virtual, not physical
  memory;

The whole change could be described in two pictures below:
before and after the change.

Some aspects of the virtual memory layout setup are not
clarified (number of page levels, alignment, DMA memory),
since these are not a part of this change or secondary
with regard to how the uncoupling itself is implemented.

The focus of the pictures is to explain why __va() and __pa()
macros are implemented the way they are.

Unused gaps in the virtual memory layout could be present
or not - depending on how partucular system is configured.
No page tables are created for the unused gaps.

The relative order of vmalloc, modules and kernel image in
virtual memory is defined by following considerations:

- start of the modules area and end of the kernel should reside
  within 4GB to accommodate relative 32-bit jumps. The best way
  to achieve that is to place kernel next to modules;

- vmalloc and module areas should locate next to each other
  to prevent failures and extra reworks in user level tools
  (makedumpfile, crash, etc.) which treat vmalloc and module
  addresses similarily;

- kernel needs to be the last area in the virtual memory
  layout to easily distinguish between kernel and non-kernel
  virtual addresses. That is needed to (again) simplify
  handling of addresses in user level tools and make __pa()
  macro faster (see below);

Concluding the above, the relative order of the considered
virtual areas in memory is: vmalloc - modules - kernel.
Therefore, the only change to the current memory layout is
moving kernel to the end of virtual address space.

With that approach the implementation of __pa() macro is
straightforward - all linear virtual addresses less than
kernel base are considered identity mapping:

	phys == virt - __identity_base

All addresses greater than kernel base are kernel ones:

	phys == (kvirt - __kaslr_offset) + __kaslr_offset_phys

By contrast, __va() macro deals only with identity mapping
addresses:

	virt == phys + __identity_base

.amode31 section is mapped separately and is not covered by
__pa() macro. In fact, it could have been handled easily by
checking whether a virtual address is within the section or
not, but there is no need for that. Thus, let __pa() code
do as little machine cycles as possible.

The KASAN shadow memory is located at the very end of the
virtual memory layout, at addresses higher than the kernel.
However, that is not a linear mapping and no code other than
KASAN instrumentation or API is expected to access it.

When KASLR mode is enabled the kernel base address randomized
within a memory window that spans whole unused virtual address
space. The size of that window depends from the amount of
physical memory available to the system, the limit imposed by
UltraVisor (if present) and the vmalloc area size as provided
by vmalloc= kernel command line parameter.

In case the virtual memory is exhausted the minimum size of
the randomization window is forcefully set to 2GB, which
amounts to in 15 bits of entropy if KASAN is enabled or 17
bits of entropy in default configuration.

The default kernel offset 0x100000 is used as a magic value
both in the decompressor code and vmlinux linker script, but
it will be removed with a follow-up change.

This is a preparatory rework to allow uncoupling virtual

Currently __kaslr_offset is the kernel offset in both
physical memory on boot and in virtual memory after DAT
mode is enabled.

Uncouple these offsets and rename the physical address
space variant to __kaslr_offset_phys while keep the name
__kaslr_offset for the offset in virtual address space.

Do not use __kaslr_offset_phys after DAT mode is enabled
just yet, but still make it a persistent boot variable
for later use.

Use __kaslr_offset and __kaslr_offset_phys offsets in
proper contexts and alter handle_relocs() function to
distinguish between the two.

65d37f163a s390/cmma: move arch_set_page_dat() to header file
In order to be usable for early boot code move the simple
arch_set_page_dat() function to header file, and add its counter-part
arch_set_page_nodat(). Also change the parameters, and the function name
slightly.

This is required since there aren't any struct pages available in early
boot code, and renaming of functions is done to make sure that all users
are converted to the new API.

Instead of a pointer to a struct page a virtual address is passed, and
instead of an order the number of pages for which the page state needs be
set.

2d1494fb31 s390/mm: make virt_to_pfn() a static inline
Making virt_to_pfn() a static inline taking a strongly typed
(const void *) makes the contract of a passing a pointer of that
type to the function explicit and exposes any misuse of the
macro virt_to_pfn() acting polymorphic and accepting many types
such as (void *), (unitptr_t) or (unsigned long) as arguments
without warnings.

For symmetry do the same with pfn_to_virt() reflecting the
current layout in asm-generic/page.h.

Doing this reveals a number of offenders in the arch code and
the S390-specific drivers, so just bite the bullet and fix up
all of those as well.

966ffbd861 s390/mm: convert pte_val()/pXd_val() into functions
Disallow constructs like this:

        pte_val(*pte) = __pa(addr) | prot;

which would directly write into a page table. Users are supposed to
use the set_pte()/set_pXd() primitives, which guarantee block
concurrent (aka atomic) writes.

e613d83454 KVM: s390: Honor storage keys when accessing guest memory
Storage key checking had not been implemented for instructions emulated
by KVM. Implement it by enhancing the functions used for guest access,
in particular those making use of access_guest which has been renamed
to access_guest_with_key.
Accesses via access_guest_real should not be key checked.

For actual accesses, key checking is done by
copy_from/to_user_key (which internally uses MVCOS/MVCP/MVCS).
In cases where accessibility is checked without an actual access,
this is performed by getting the storage key and checking if the access
key matches. In both cases, if applicable, storage and fetch protection
override are honored.

92638b4e1b mm: arch: remove indirection level in alloc_zeroed_user_highpage_movable()
In an upcoming change we would like to add a flag to
GFP_HIGHUSER_MOVABLE so that it would no longer be an OR
of GFP_HIGHUSER and __GFP_MOVABLE. This poses a problem for
alloc_zeroed_user_highpage_movable() which passes __GFP_MOVABLE
into an arch-specific __alloc_zeroed_user_highpage() hook which ORs
in GFP_HIGHUSER.

Since __alloc_zeroed_user_highpage() is only ever called from
alloc_zeroed_user_highpage_movable(), we can remove one level
of indirection here. Remove __alloc_zeroed_user_highpage(),
make alloc_zeroed_user_highpage_movable() the hook, and use
GFP_HIGHUSER_MOVABLE in the hook implementations so that they will
pick up the new flag that we are going to add.

c62da0c35d mm/vma: define a default value for VM_DATA_DEFAULT_FLAGS
There are many platforms with exact same value for VM_DATA_DEFAULT_FLAGS
This creates a default value for VM_DATA_DEFAULT_FLAGS in line with the
existing VM_STACK_DEFAULT_FLAGS.  While here, also define some more
macros with standard VMA access flag combinations that are used
frequently across many platforms.  Apart from simplification, this
reduces code duplication as well.
