The driver that used the 'nodisconnect' parameter was removed in
commit 565bae6a4a ("[SCSI] 53c7xx: kill driver"). Related documentation
was cleaned up in commit f37a7238d3 ("[SCSI] 53c7xx: fix removal
fallout"), except for the remaining two mentions that are removed here.

The support for CONFIG_MCA is being removed, since the 20
year old hardware simply isn't capable of meeting today's
software demands on CPU and memory resources.
This commit removes the MCA specific SCSI drivers, and the
MCA specific portions of code in dual role ISA/MCA drivers.
Also, the MCA specific SCSI documentation is removed.

Move SCSI parameters from kernel-parameters.txt to their own text file.
