Use a table for the delivery systems. The table is organized
by the type (cable, satellite, terrestrial) and shows what
standards are not fully implemented.

The ATSC M/H specific properties are not properly documented.
This became crearer when converting the existing data into
tables and adding cross references.
For now, just add placeholders, as a further investigation
about the meaning of each parameter is required.

There are lots of enums that are defined at dvbproperty.
Add xrefs for each entry there. This makes the hyperlinks at
frontend.h to go directly to the right documentation.

The notes there are somewhat confusing and assumes that the
reader would have read the DVBv3 way. This is not true anymore,
as the DVBv3 is now on a separate section that is marked as
deprecated.
So, cleanup the notes.
While here, add a note about using libdvbv5, instead of using
the DVBv5 API directly.

Instead of having two refentries, merge them into just one,
like what's done with other similar ioctls at V4L2 side.

In comments and in the documentation, the units of properties marked
with the FE_SCALE_DECIBEL scale are specified in terms of 1/1000 dB
or 0.0001 dB. This is inconsistent, however, as 1/1000 is 0.001,
not 0.0001.
Note that the v4l-utils divide the value by 1000 for the signal
strength suggesting that the 1/1000 is correct.
Settle on millidecibels, ie. 1/1000dB or 0.001dB.

The DocBook documentation is incorrect: on ISDB-T, interleaving
time is always a power of 2. Fix it and provides a table showing
the actual interleaving length for each mode.

The DVBv3 statistics parameters are limited on several ways:
- It doesn't provide any way to indicate the used measure,
so userspace need to guess how to calculate/use it;
- Only a limited set of stats are supported;
- Can't be called in a way to require them to be filled
all at once (atomic reads from the hardware), with may
cause troubles on interpreting them on userspace;
- On some OFDM delivery systems, the carriers can be
independently modulated, having different properties.
Currently, there's no way to report per-layer stats.
To address the above issues, adding a new DVBv5-based stats API.
While here, correct inner code nomenclature on a few places.

what delivery systems currently support it (DVB-T2, DVB-S2,

After thinking twice, I ended up adding own value for AUTO
interleaving instead of using NONE.
API minor number is not needed to increase as that patch should
be the same Kernel as interleaving parameter is initially added.

On ISDB-T, each layer can have its own independent modulation,
applied to the carriers that belong to the segments associated
with them. So, there's no sense to define a global modulation
parameter.

The ISDB-T differs on its way to implement the hierarchical
transmissions: instead of using a low-priority/high-priority
FEC codes, it does that by using different layers, each layer
with their groups of segments. So, those parameters don't make sense
for ISDB-T.

The units for DTV_FREQUENCY are kHz for satellital delivery systems
(DVB-S/DVB-S2/DVB-TURBO/ISDB-S). Fix it at the API spec.

Annex C, split them into two separate ones. This helps to support
devices that only support Annex A.
