Add an instance parameter to amdgpu_dpm_enable_vcn() function, and change
all calls from vcn ip functions to add instance argument. vcn generations
with only one instance (v1.0, v2.0) always use 0 as instance number. vcn
generations with multiple instances (v2.5, v3.0, v4.0, v4.0.3, v4.0.5,
v5.0.0) use the actual instance number.
v2: remove for-loop in amdgpu_dpm_enable_vcn(), and temporarily move it
to vcn ip with multiple instances, in order to keep the exact same logic
as before, until further separation in next patch.

Update the *handle to amdgpu_ip_block ptr for all
functions pointers of hw_fini.
Also update the ip_block ptr where ever needed as
there were cyclic dependency of hw_fini on suspend
and some followed clean up.

Avoid returning an uninitialized value if we never enter the loop.
This case should never be hit in practice, but returning 0 doesn't
hurt.
The same fix is applied to the 4 places using the same pattern.

Simplifies the code so that all VCN versions will get the firmware
name from `amdgpu_ucode_ip_version_decode` and then use this filename
to load microcode as part of the early_init process.
