d636a932b3 habanalabs: clean MMU headers definitions
During the MMU development the MMU header files were left with unclean
definitions:

- MMU "version specific" definitions that were left in the mmu_general
  file
- unused definitions

This patch attempts, where possible, to keep definitions that can serve
multiple MMU versions (but that are not tightly bound with specific MMU
arch) in the mmu_general header file (e.g. different definitions for
number of HOPs).

Otherwise, move MMU version specific definitions (e.g. HOPs masks and
shifts) to the specific MMU version file.

a1b838adb0 habanalabs: fix possible deadlock in cache invl failure
Currently there is a deadlock in driver in scenarios where MMU
cache invalidation fails. The issue is basically device reset
being performed without releasing the MMU mutex.
The solution is to skip device reset as it is not necessary.
In addition we introduce a slight code refactor that prints the
invalidation error from a single location.

dd08335fb9 habanalabs: fix debugfs device memory MMU VA translation
The translation in debugfs of device memory MMU virtual addresses was
wrong as it did not take into consideration the fact that the page
sizes there can be not power of 2.

89aad770d6 habanalabs: fix nullifying of destroyed mmu pgt pool
In case of host-resident MMU, when the page tables pool is destroyed,
its pointer is not nullified correctly.
As a result, on a device fini which happens after a failing reset, the
already destroyed pool is accessed, which leads to a kernel panic.
The patch fixes the setting of the pool pointer to NULL.

0f37510ca3 habanalabs: fix mask to obtain page offset
When converting virtual address to physical we need to add correct
offset to the physical page.

For this we need to use mask that include ALL bits of page offset.

15097e9338 habanalabs: fix debugfs address translation
when user uses virtual addresses to access dram through debugfs,
driver translate this address to physical and use it
for the access through the pcie bar.
in case dram page size is different than the dmmu
page size, we need to have special treatment
for adding the page offset to the actual address, which
is to use the dram page size mask to fetch the page offset
from the virtual address, instead of the dmmu last hop shift.
